Inpatient vs Outpatient Psych Rotations: What Your Program Will Accept

Psychiatric-Mental Health Nurse Practitioner programs typically divide clinical hours between outpatient and inpatient psychiatric settings. Understanding which counts and which your program mandates is critical to avoiding a rejected rotation.

Outpatient psychiatric settings include private-practice psychiatric medicine (the most common PMHNP rotation site), community mental health centers, integrated primary care behavioral health (where a PMHNP embeds within a primary care practice), and telehealth-based psychiatric practices. Most PMHNP programs accept telehealth hours if the supervising preceptor is actively seeing patients during your shift, though some require a minimum in-person component. Partial hospitalization programs (PHP) and intensive outpatient (IOP) also count.

Inpatient psychiatric settings include acute psychiatric hospitals, hospital-based behavioral health units, and addiction-medicine inpatient programs. Many PMHNP programs require a minimum of inpatient hours, though the trend is shifting toward outpatient-heavy training given the specialty’s growing community-based care model.

DEA certification matters too. If you are in a later clinical semester where you will be writing prescriptions under supervision, your preceptor needs DEA registration. PMHNP rotations heavily involve controlled-substance prescribing (stimulants for ADHD, benzodiazepines for anxiety, buprenorphine for opioid use disorder), so preceptor DEA status is a hard requirement for most programs.

State-specific practice is a wrinkle for telehealth preceptors. APRN Compact states allow preceptors to supervise across state lines, but non-compact states may require the preceptor be licensed in your state. Your school program office usually sets the rule.
