Skilled nursing facilities are the most common site. You'll see long-term residents, post-acute rehab patients and end-of-life needs, which builds complex polypharmacy management, pressure injury prevention, advance care planning and functional assessment. Assisted living is less intensive: residents are more independent, so you focus on preventive care, chronic disease management and coordination with family caregivers, and programs often pair it with SNF hours to reach the total.
Geriatric primary care clinics serve adults 65 and over, and many programs treat them as the core AGPCNP rotation. Visits run 45 to 60 minutes and focus on cognitive and functional assessment, deprescribing conversations and care coordination. PACE (Programs of All-Inclusive Care for the Elderly) adds care coordination and social determinants exposure for frail elders living at home, and hospice or palliative rotations may count depending on your program.
