NP Student Liability Insurance

NP student liability insurance is malpractice coverage for the care you give as a student during clinical rotations. Whether you need to buy your own policy depends on your program. Some schools cover every student under a group policy, while others require each student to carry an individual one. Either way, your clinical site will want proof of coverage before you see patients. Insurance is one of the quicker items on your clinical checklist once you know what your program asks for.

Do you need your own NP student liability insurance?

You need your own policy when your program or your clinical site requires one. Your student handbook or clinical policies will tell you which, usually in a section on malpractice or liability insurance. Schools handle coverage in one of two ways:

  • Your school buys a group policy. The University of Pittsburgh, for example, buys a “blanket” policy that covers every student who needs it, with limits of $1,000,000 per medical incident and $3,000,000 aggregate. Pitt bills students in health care courses for it at registration.
  • You buy an individual policy. Berkeley College’s MSN program requires students to buy their own with at least $1,000,000 per occurrence and $3,000,000 aggregate, and tells them to “choose Nurse Practitioner Student insurance.”

What does a school policy cover, and where does that coverage end?

A school policy covers the clinical work your program assigns and approves. Pitt’s blanket policy, for example, applies “provided the curriculums are under the University and at our direction.” Anything outside your curriculum is outside the policy. Watch for these gaps:

  • Your RN job. Shifts for your employer aren’t part of your curriculum.
  • Volunteer work. Elon University’s PA handbook says its school coverage “WILL NOT be in effect” for outside employment or for volunteer work unless the program assigned it.
  • A site your program hasn’t approved. School policies such as Texas Tech’s cover students only in settings the program has approved, so get that approval before your first day.
  • Limits the school sets. NSO notes that coverage from your school or facility “may offer some coverage. However, there may be limits, which can leave unexpected expenses for you to pay.”

You can carry a personal policy even when your school covers you. Texas Tech tells its students that, depending on their finances, the school’s coverage “may or may not be adequate.”

Why do clinical sites ask for proof of coverage?

Clinical sites want you insured before you treat any of their patients, and they usually write that requirement into the affiliation agreement they sign with your school. Our guide explains what an affiliation agreement is and who signs it. Texas Tech’s PA handbook states it plainly: “All clinical facilities require students to have professional liability insurance.”

University templates show the same pattern. The UMass Lowell affiliation template requires the college to carry $1,000,000 per occurrence and $3,000,000 aggregate and to show proof “prior to the students commencing any patient care activity.” In the ASU-Newport template, the school must require every participating student to carry the same limits.

Claims against NP students are uncommon but costly. CNA and NSO’s 2022 nurse practitioner claim report found that claims involving NP students “tend to be infrequent,” yet they averaged more than $200,000, counting settlements and defense costs. They typically came from “inadequate supervision of the treatment of high acuity patients.”

What do limits like $1,000,000/$3,000,000 mean?

The first number is the most the insurer will pay for a single claim. The second, called the aggregate, is the most it will pay for all your claims during one policy term, which is often one year. So a $1,000,000/$3,000,000 requirement means your policy needs at least $1 million per claim and at least $3 million aggregate. Here’s what several insurers publish for their student policies:

  • HPSO student policy: up to $1,000,000 each claim and $3,000,000 annual aggregate.
  • NSO student nurse policy: up to $1 million each claim and $6 million annual aggregate.
  • CM&F NP student policy: up to $1 million per claim and $6 million aggregate.
  • Berxi NP student policy: limits that typically range from $500,000/$1 million to $2 million/$6 million, so choose a level that meets your requirement.

Also check how the policy handles legal fees. HPSO and Berxi both pay defense costs in addition to the limits, so those fees don’t reduce the amount available for a settlement.

Occurrence or claims-made: which policy type do you need?

An occurrence policy is the simpler choice for a student. It covers incidents that happened while the policy was active, even if a patient files the claim years after you graduate.

A claims-made policy covers a claim only if someone files it while the policy is still active. CM&F gives the example of a policy that has expired when a former patient sues six months later: a claims-made policy “would no longer cover that claim” unless you’d bought an extended reporting endorsement, also called tail coverage.

Since you’ll move to a new policy after graduation, occurrence coverage keeps your student rotations covered with no tail coverage to buy. CPH lists an “Occurrence Form” on its NP student page, and Pitt’s school policy is occurrence coverage too. Ask your program whether it requires one type or the other.

How much does NP student malpractice insurance cost?

An NP student policy can start around $140 a year. That’s the starting price Berxi publishes, and of the student insurers we checked, Berxi is the one that lists a price specifically for NP students. The others quote through an online form that takes a few minutes.

You’ll also see lower prices for pre-licensure students. NSO lists $44 a year for its student nurse policy, and HPSO says most students can get coverage “for as little as $43 per year.” An NP student policy is a different product because, as Proliability puts it, “Your clinical work as a nurse practitioner student may include duties outside the scope of your RN license.” Choose the NP student option, the way Berkeley College tells its students to.

Berxi notes that NP student policies “vary in cost depending on a number of factors,” so get your NP student liability insurance quote for the exact limits your program requires. If your school covers you, the cost may already be part of your fees, as it is at Pitt.

How do you buy NP student liability insurance?

Many insurers sell student policies online, and you can often buy one and download proof of coverage the same day.

  1. Get your program’s requirement in writing. Note the limits, any required policy type, and who should receive your proof.
  2. Choose an NP student insurance policy. When the quote form asks for your profession, select nurse practitioner student rather than student nurse.
  3. Match or exceed both limits. Check the per-claim and aggregate numbers against your requirement.
  4. Set your start date. Coverage should begin on or before your first clinical day and run through your last. Berxi, for example, says a policy can go into effect “any day you’d like, including today.”
  5. Download your certificate of insurance. Send it to your program and your site, and keep a copy with the documents you’ll bring on the first day of your clinical rotation.

What should you ask your program before your first rotation?

Send your clinical coordinator a short email with these questions:

  • Does the school’s policy cover me, or do I need to buy my own NP student liability insurance?
  • What limits do you require per claim and in aggregate?
  • Do you require occurrence coverage?
  • Does my site need to be listed on my certificate, or added as an additional insured?
  • When is proof due, and who should receive it?
  • If I find my own preceptor, when does coverage start at that site?

That last question matters when your program expects you to find your own site. Many programs cover you only at sites they’ve approved, so ask before you commit to a preceptor. NP student liability insurance questions

Do nurse practitioners have to carry liability insurance?

As a student, your program and your clinical sites decide whether you need your own policy. After you’re licensed, the answer depends on your state and your employer, so check with your state board of nursing and any employer you’re considering.

How much is $1,000,000 liability insurance a month?

Student policies are priced by the year, and many include $1 million per claim. NSO’s student nurse policy, with up to $1 million per claim, costs $44 a year. Berxi says its NP student policies can start around $140 a year, which works out to about $12 a month.

How often do NPs get sued?

Claims against NP students are infrequent, according to CNA and NSO’s 2022 claim report. It reviewed 232 closed claims against NPs, NP-owned practices and NP students that paid $10,000 or more, and the average claim cost $332,137 in total.

Does my RN malpractice policy cover clinicals?

It may not, so ask your insurer before you rely on it. Your NP clinical work “may include duties outside the scope of your RN license,” as Proliability points out. Berxi and Proliability both offer NP student policies that cover your RN work too, though Proliability notes this depends on state availability.

What happens to my student policy when I graduate?

Many insurers help you move to a policy for your NP role. HPSO says student coverage may extend to your professional duties at no extra premium “for six months or upon your next renewal, whichever comes first.” NSO offers new graduates a discount on their first year’s premium, and CM&F’s student NP policyholders are eligible for new graduate NP rates.

Do PA students need liability insurance?

Yes. Clinical sites expect PA students to carry coverage too, and some PA programs, such as Elon’s and Texas Tech’s, cover their students under a school policy of $1,000,000/$3,000,000.
